Fall 2015 Flyer - Info for wrong Return Address

Please be aware if you mailed in your registration for the fall 2015 conference, it may possibly be returned due to wrong return address.

If this happens, please bring the registration with you to the conference or you my take it to Tanner Clinic to Sherri McMillian in the ENT depart.
